This may not be exact but I think it's pretty close.

One note on the bridge: even though the chord chart doesn't show it (for reasons of space)the bars of FM7 (2nd & 3rd lines of bridge) should look like:

--ll---/------3------
(FM7 FM7 GbM7 GM7) with the 1st FM7 getting 2 beats the 2nd FM7 and the other 2 chords played as a quarter-note triplet.

It might look complicated but what you are really doing is just moving the F major 7th chord up three frets, one fret at a time to get to the Ab major 7th.
