Put a pulley between the lever and the keyhead and take the cable from the lever around the pulley and back to the changer.

Are you looking for a way to lower with an all-pull cable guitar?

If so, have your tuning keys pivot with a pull like this where the tuning keys are mounted on a plate with a finger that pokes through to the bottom on the cabinet. The tuner pivots on the tuning key shaft that is passing through the keyhead, with a hole hopefully lines with an oilite bushing:

Rods are used here, but they are pull rods, so a cable would do the same thing. This is like a Bigsby pivoting tuning key system.

Or you do the Bigsby, Wright and Sho-Bud trick of bellcranks that are mounted BEHIND the fingers -- between the fingers and the endplate:

Red Bellies, Bigsbys and a lot of other guitars.
