Fender 400-800-1000-2000 Logo Question

This may not be the place to post this question so if that is the case, apologies in advance.

1) Is the Fender tag that goes on the front of the body of the 400-800-1000-2000 guitars the same for all those models and all the years it was made?
2) Does anyone know of a source for a replacement, used, NOS or remake?
3) How did they attach?
4) Would someone be willing to take a digital photo of theirs so when I do find one, I know where to mount it?
