It is configured extended e9th . Serial #122-0008 . I dont know the year but perhaps the serial will help.

Its one of the later models of a Slimline evidenced by the removeable Rear plate secured by 6 Screws. Nice looking S12 Guitar with 5 X 5 and from What I see its triple raise, double lower. Has all the features anybody could want.Pre-Drilled and Pre Machined, Expandable up to 11 pedals X 10 Knees. If I had The Bucks, I would have it sitting beside my D210
Nice looking guitar! It's not a SlimLine because of the cast endplates. I don't remember if we started adding the back plate before the switch to the extruded endplates on the SlimLine, but apparently so, because it looks factory.
Based on the low serial number, I would guess that this is about a 1977 or 1978 model.

Those are the screws for the block that the reversing part of the knee lever mounts to . This guitar does not have the removable panel. Look closely and you can see the bushings that screw into the rear panel.
It is a great playing and sounding guitar .
