Found this chart on the web. How accurate is it?

Never really needed to learn it, back several years ago I learned the basic 1-4-5 but never got into anything deeper than that. I'm in a situation now where it would be a lot easier to use this.

That's quite good, actually. When I do charts in Word on the computer, I superscript the 7 for 7th chords. I can't make it show here, but the "7" would be smaller and to the upper right of the number for the chord. For minors, I use "-" rather than "m". It looks like 2- rather than 2m. Either one seems to work fine. The parentheses around a split measure (1 5) seems to be preferred by most players. This article didn't touch on augmented or diminished symbols, or 1st & 2nd ending, codas and such, but the material covered will get most of your charts very easy to read.

Augmented is usually notated by adding a + after the number (5+) and diminished by using a small circle in the upper right hand corner of the number indicating the chord. (Like notating temperature)I don't have that small circle on my keyboard. Half diminished (m7b5) uses the same circle with a line through it. (ø) Also accepted for split bars are the numbers in the split bar underlined. 1st and 2nd endings are often treated as they are in written notation, with the bracket containing the appropriate number above the phrase. I will always write the charts fairly specific to the musicians that will be on the session. It helps to know them and how they prefer to read, since the Nashville number system is still not a perfect language, but it gets the job done better than anything else.
